Withdrawal from DASS Participation Instructions

Withdrawal from participation in the Dashboard Alternative School Status (DASS) program.

The Withdrawal from DASS Participation Form is for use by any school currently participating in DASS. A school that withdraws from DASS participation will be held accountable under state indicators and standards developed for traditional (non-alternative) schools, and will not be eligible for participation in DASS for three years.

Completing the Withdrawal from DASS Participation Form

  1. Complete all fields on the Withdrawal from DASS Participation Form (The Withdrawal from DASS Participation Form is an online survey and closes on the submission deadline, October 6, 2017.)
  2. Submit the Withdrawal from DASS Participation form online

Submitting Supporting Document

Obtain signatures of the school principal, county or district superintendent or charter school administrator, and board president on the Withdrawal from DASS Certification Form.

Submit the completed Withdrawal from DASS Certification Form to:
